Weight Watchers Reimbursement Criteria
Once every two years, a weight watchers participant could be eligible for up to a $75 reimbursement on the enrollment fees they have paid to weight watchers when participating in one of these options:
- One 13-week session for the AT WORK Program, or
- One 14-week session for the "Traditional" WeightWatchers Meetings, or
- One 14-week session for the WeightWatchers on-line program
Cash reimbursements are a taxable benefit and will be included in your taxable income as reported by the State of Montana on your W-2.
Criteria
So here is how it works. The employee must meet the following 4 criteria:
1st Criteria - Weight
Your beginning weight must be at least: 10% over the maximum weight for your age (see WeightWatchers chart).
OR you may qualify with a recommendation for weight loss from your Health Screening results, from RBH, our EAP provider, or a written prescription from a physician, for weight loss due to diabetes, high blood pressure, or high cholesterol.
2nd Criteria - Attendance
You must attend at least 75% of the classes offered.
3rd Criteria - Achievement
You must achieve the 10% weight loss goal set in advance by the Weight Watchers instructor. This means that you need to meet the 10% goal but you can take longer than 13-14 weeks, so you could go until you reach it, but only the initial 13 or 14-week period is used to compute your reimbursement.
4th Criteria - Exercise
You must document your exercise included in your program and you need to do some form of exercise three times a week and keep a journal of it.
Reimbursement
This is what you need to file for reimbursement:
Initially, you will need to e-mail the Weight Watchers coordinator and give her your name, address, telephone, state agency, e-mail address, start date and which program you are going to participate in.
After you have met the 4 criteria listed above and you want to submit your request for payment, you will need to send a copy of the Weight Watchers Membership Book, the Exercise Journal, a signed 10% Goal Certificate and your employee ID number to the coordinator. If you chose the online option, print off copies of what you paid and a record of your weight loss. (Everything will be kept confidential.)
Please send all your above information to the Weight Watchers Coordinator, Janet Thornburg at PO Box 200127 Helena, MT 59620.
Participants need to pay upfront for the classes they are attending. Upon completion you will be reimbursed within 90 days of submitting all the documented information to our coordinator. Since the reimbursement is taxable, it will be process through the Sate Payroll System as an off-cycle check, separate from the bi-weekly pay checks.
